Dear Universal Karate & Fitness Parents,

Thank you for signing up your child for Universal Karate & Fitness After Care Program. The staff and I are very excited about all the fun activities that we have planned for the school year. At Universal Karate & Fitness we want a "safe and fun experience" while helping your young student achieve their best while in school. We genuinely hope that your child's time with us is one that will create The vision at Universal Karate & Fitness is to provide an experience where all kids can create memories with new friends and new experiences. We want to have an environment where members can learn new things, while feeling safe and successful. We have a wide variety of programming happening at Universal Karate & Fitness ranging from; sports, photography, science experiments, and arts and crafts. As they run, laugh, smile, and play, it is our hope that members will learn that each moment in life is truly something to remember. Enclosed in this registration packet you will find all the necessary information to ensure your child's enjoyment and safety at Universal Karate & Fitness. If you have any questions about any of the contents of this packet, do not hesitate to contact one of our senior staff The Universal Karate & Fitness staff has a heart for kids. It is our mission to serve you as best as we can. Please let us know if your family has any special needs, or if there is anything that we can do to make this experience the best that it can be for your kids. If you would like to sign up to volunteer at any of our field trips please speak with Mr. Keith. Any donations in the form of games, art supplies or snacks are welcomed. STUDIO RULES

UNIVERSAL KARATE & FITNESS will implement "STUDIO RULES." These rules are developed to maximize customer service in order to keep our studio clean and safe for our students and patrons.

1. All children's belongings (shoes, socks, bags, and other personal items) will be placed in assigned cubby holes.

2. ONLY STUDENTS are allowed onto the mat to honor tradition and respect to their training facility.

3. Children will take shoes and socks off upon entering onto the mat play area and place them on the shoe racks and jackets in the cubby holes. Students WILL NOT wear pants/ hoodies underneath their uniforms. T-shirts and sports tops are allowed.

4. All packed lunches MUST be checked in with faculty during drop off (for breaks, closings and early releases

5. Chewing gum is PROHIBITED.

6. Children are not allowed to touch any books, games, or the television without permission of staff.

7. Cellphones, tablets and other electronic devices are prohibited without signed permission slip and liability wavier from parent/guardian/director.

8.Over the counter and prescription medication are PROHIBITED. Faculty cannot administer to children.

9. Guests and parents are to view classes from the guest room/area.

10. Children MUST be picked up by 7:30p.m. Monday-Friday. ($1.00 penalty for every minute child stays past pick-up time

11. KARATE CLASSES: Wednesday & Saturday classes ALWAYS require sparring gear.

PAYMENT POLICIES

Hello Parent of Universal Karate & Fitness Afterschool Program,

We are glad to have you all with us for this new year! We are dedicated to providing exceptional Before Care, Afterschool Care and Karate Classes to you and your family. As we move more into 2019, we would like to send you a friendly reminder that will help us continue to serve you and your family with the exceptional quality you've come to expect, and we enjoy delivering.

1. Registration Fee: The registration fee for each child is due upon enrollment and is NON-REFUNDABLE and NON-TRANSFERABLE.

2. Special Pricing: The $75 per week special and any other special pricing is for 'CARDS ON FILE' ONLY, if you do not wish to place your card on file the normal market price of $95 per week will apply.

3. Payment: Payment is due the Friday before services are rendered There is a $10 late fee PER DAY for any payments 24 hours past due (Tuesdays of each week), when staff has been notified. A $10 fee will be applied if your card is declined for ANY reason.

4. School Pick-Up: If an account is not current and no attempt to contact, or return attempted contacts with the staff at Universal Karate & Fitness has been made, your child(ren) will not be picked up from school until the account is brought current and

5. Late Parent Pickup: A late pick up fee is $2.00 per minute. The late fee will be applied to your account.

6. Canceling & Scheduling Pickup: We understand that emergencies happen. When able, we ask that we get any canceled pickups be made before 1pm the day of and any scheduled pickup be made by 1pm the day before. If the deadline is not met there will be a $5 fee applied to your account.

7. Cards On File: As of March 1, 2019. Universal Karate & Fitness will no longer be accepting "pre-paid" cards for our "card on file" system. Our system does not allow them to save, so any card that is placed "on file" must be a credit card or debit card.
